Please print and complete ALL of the required forms
on this page and return to American Academy. Note
that some of the forms are eForms - you may fill
the eForms out electronically and then print.
You must bring in ALL items and completed forms
listed on the New Student Registration Checklist in
order for us to process your registration.

REQUIRED Student Registration Forms (complete one per student)
Use this checklist to make sure you have all of the required information. All items on this checklist must be completed before we can process your registration.
Registration form required by the school district. Please fill this form out electronically and then print.
Used to request records from your student's previous school (applies to first grade and up only).
Acknowledgement that you will be moving from your neighborhood school to American Academy.
If your student is transferring from a Douglas County School the Administrative Transfer forms needs to be signed by the school he/she is leaving before he/she can be enrolled in American Academy.
REQUIRED Family Registration Forms (complete one per family)
Required to access our parent portal.
Acknowledgement that support at home, homework time and family volunteer hours are required by American Academy.
We must have this form on file so that you can drive on a field trip, etc. Print the form out, complete, and attach vehicle insurance information as indicated on the form.
OPTIONAL Registration Forms (complete only those that apply)
Monthly payment setup for full-day kindergarten tuition.
Tuition agreement for full-day kindergarten.
Use this form to indicate residency status if you are in a temporary living situation.
This agreement must be completed for kids who will be walking to and from school on a regular basis.
Monthly payment setup for the Kindergarten Lunch Bunch program. The Lunch Bunch program is for half-day kindies (AM and PM). Your kiddo can stay or hour later or come an hour earlier and have lunch and recess at school (and you can have an extra hour of freedom!).
Fill this form out to apply for assistance with American Academy paid programming, such as hot lunch, uniforms, field trips, fees, etc.
Form for parents to fill out if they would prefer to make payments in increments. EFT payments only.
